Near Miss Reporting
A near miss is a warning that the system allowed an incident to almost happen. Reporting near misses is not about blame. It helps the team identify weak controls before someone is injured. Good reports describe what happened, where it happened, what could have occurred, and what immediate correction was made. Supervisors should thank employees who report near misses because they are helping prevent the next serious event.
